Health Questionnaire Download

• After confirming your reservation, register at the Health Promotion Center located on 10F of outpatient department.
• Arrive at the center on time for your check-up. (Between 8 am ~ 9 am)
• Submit your health questionnaire and stool sample container.
• Make the payment for your check-up.

• Go to the changing room and change into the examination gown and slippers.
• Valuables can be kept separately.
• General health check-up program will take approximately 2~3 hours. (Additional examinations can increase the time required)

• The results of general check-up will be available 7~10 days after the examination.
• You can consult our professionals regarding your results through visits, posts, email or phone calls.
• Consultation through phone call is available between 2~5 pm on weekdays.
